SPX Physical & Consent Form

Middle School Physical & Consent Form

The Georgia High School Association (GHSA) requires an annual physical for every student athlete. In addition, St. Pius X requires that every middle school student athlete who participates in the St. Pius X athletic program must also have an annual physical. Because GHSA mandates the physical form we use, it is beneficial that our physicals are done at the same time. The St. Pius X Athletic Association will sponsor physicals on Tuesday, May 13, 2008 for a non-refundable fee of $30.00. The physical will cover student athletes for all of next year, as well as any sports team camps they attend this summer. Please note that physicals performed prior to April 1, 2008 are not valid for next year.

All student athletes, both high school and middle school must have an up-to-date physical on file at St. Pius before they can try out, practice or participate in any sport.

The physicals will begin at 3:15 p.m, and the last physical should be completed by 5:30 p.m.   For the exam, please have your student athlete dress in gym shorts and a t-shirt.

For current student athletes at St. Pius X, physical forms are available from the Athletic Office, or you can download a copy of the SPX Physical & Consent Form. The Medical History side of this form must be completed and signed by a parent or legal guardian. The consent to participate in athletics section (found on the third page of the form) must also be signed by a parent or legal guardian.   Please return completed forms to Dale Stanley in the Athletic Office by Friday, May 2, 2008.  Along with the form, please enclose your check for $30.00 made payable to the St. Pius X Athletic Association.

For middle school student athletes, please look for a letter and physical form in the weekly folder sent home with your child, or you can download a copy of the Middle School Physical & Consent Form. The Medical History side of this form must be completed and signed by a parent or legal guardian. The consent to participate in athletics section (found on the third page of the form) must also be signed by a parent or legal guardian.   Please return completed forms to Dale Stanley in the Athletic Office by Friday, May 2, 2008.  Along with the form, please enclose your check for $30.00 made payable to the St. Pius X Athletic Association.

If you decide to have your student athlete's physical done by your own doctor, you MUST take the St. Pius X Medical History and Physical Examination form to your doctor.  This is the only acceptable form.   For further information, please contact Dale Stanley in the Athletic Office at 404-636-7751 ext 266.
