Student Physical Forms
SMART HEART SCANS 2015
Each year every St. Pius X student-athlete is required to undergo a physical. The vast majority of the student-athletes take that very thorough physical here at St. Pius X in May. There are, however, certain heart conditions that cannot be detected by regular sports physicals. Over the past several years, there have been several deaths throughout the United States and here in the state of Georgia resulting from an athlete's heart condition that could only be detected by an Echocardiogram (ECHO). An ECHO is designed to screen for hypertrophic cardiomyopathy, a heart condition that reduces the heart’s ability to pump blood effectively to the body. The ECHO and its interpretation by a cardiologist routinely costs $800 to $1500, thus making the procedure cost prohibitive for many.
Saturday, August 15, St. Pius X will offer echocardiograms through Smart Heart Scans (please keep in mind this is not required for a student-athlete to participate on a team at St. Pius X). The process takes approximately 30 minutes from check-in to conclusion. If you wish to register your child, please download the registration form below and mail the completed form along with your check for $70 made out to Smart Heart Scans, attention Gary Schmitt by Tuesday, August 4.
If you have any questions, please do not hesitate to contact Gary Schmitt at 404-636-7751 ext. 267.
The Georgia High School Association (GHSA) requires an annual physical for every student athlete. In addition, St. Pius X requires that every middle school student athlete who participates in the St. Pius X athletic program must also have an annual physical. Because GHSA mandates the physical form we use, it is beneficial that our physicals are done at the same time. The St. Pius X Athletic Association will sponsor physicals on Thursday, May 14, 2015 for a non-refundable fee of $30.00. The physical will cover student athletes for the entire 2015-2016 school year, as well as any sports team camps they attend this summer. Please note that physicals performed prior to April 1, 2015 are not valid for the school year 2015-2016.
All student athletes, both high school and middle school must have an up-to-date physical on file at St. Pius X before they can try out, practice or participate in any sport.
The physicals will begin at 3:15 p.m, and the last physical should be completed by 5:30 p.m. For the exam, please have your student athlete dress in gym shorts and a t-shirt.
The physical form package can be downloaded from the St. Pius X website (www.spx.org/athletics/Student Physical Forms). Or you may contact Dale Stanley in the Athletics Office at 404-636-7751, extension 266 or by email at email@example.com to obtain a form.
The package includes the Student/Parent Concussion Awareness Form. This form must be read and signed by both the student and the parent/guardian.
The Medical History section of this form must be completed and signed by a parent or legal guardian. The St. Pius X Emergency Contact Information and Parental Consent section (found on the first page of the form) must also be completed and signed by a parent or legal guardian.
If your student athlete is having his or her physical done at St. Pius X on 05/14/15, please return completed forms to Dale Stanley in the Athletic Office by Tuesday, May 5, 2015.
Beginning this year, if your child is a St. Pius X student, your FACTS account will be charged $30.00 for the physical.
If your child is not a student at St. Pius X, please enclose your check for $30.00 made payable to the St. Pius X Athletic Association.
If you decide to have your student athlete's physical done by your own doctor, you MUST take the St. Pius X Medical History and Physical Examination form to your doctor. This is the only acceptable form. For further information, please contact Dale Stanley in the Athletic Office at 404-636-7751 ext 266.
